Have an unused Praxis voucher? Help another student by donating it!
Step 1: Forward the original voucher email to jametrice.johnson@sus.edu
→ Use “Donating Voucher” as the subject line.
Step 2: Complete the Voucher Donation Form press (click here) to help us track donations.

Please note: The issuance of a voucher is not guaranteed and is based on your current Praxis status, study habits, demonstrated need, and overall preparedness.
Individualized Praxis Plan (IPP) Fillable (3).pdfBefore requesting a voucher, students must complete an IPP. This plan helps outline your goals and timeline for Praxis success. It must be signed and reviewed by Ms. Johnson. IPPs are valid for one semester.
After your IPP is signed by yourself and Ms.Johnson complete this form.. IPPs are valid for one semester.
Only complete this form if you have a signed IPP on file for the current semester. Voucher requests without an approved and signed IPP will not be processed.
